Why corneal problems are so often misdiagnosed

The cornea is the clear dome at the front of the eye — about half a millimetre thick, entirely without blood vessels, and one of the most densely innervated tissues in the human body, which is why the smallest scratch on it hurts so much. It contributes roughly two-thirds of the eye's total focusing power, so its shape and clarity determine the quality of everything you see.

Corneal and ocular surface disease is probably the most commonly mismanaged area in general eye practice, because the symptoms are so generic. Burning, grittiness, redness, watering and fluctuating vision are the presenting complaints of dry eye, allergic conjunctivitis, blepharitis, early keratoconus, contact lens overwear and computer vision syndrome alike. They are treated interchangeably with lubricants and antibiotic-steroid combinations, and the ones that turn out to be steroid-responsive get prescribed steroids repeatedly — a route that leads to steroid-induced glaucoma and cataract.

Delhi's environment makes all of this worse. Sustained high particulate pollution, extreme dryness in winter, high pollen and dust loads, and long air-conditioned working hours combine to produce an unusually high burden of evaporative dry eye and allergic eye disease in this city, in patients far younger than the textbooks describe.

The way out is measurement. Corneal topography identifies keratoconus before it distorts vision noticeably. Tear break-up time and Schirmer's testing distinguish evaporative dry eye from aqueous deficiency, which need different treatments. Specular microscopy counts endothelial cells and tells you whether an eye will tolerate intraocular surgery. Guessing between these is what keeps patients cycling through drops for years.

Dry Eye Disease

Far more than a lack of tears. Most cases in Delhi are evaporative, caused by meibomian gland dysfunction where the oil layer of the tear film fails and tears evaporate too quickly. Treatment depends on the subtype: lid hygiene and warm compresses, preservative-free lubricants, omega-3 supplementation, cyclosporine or lifitegrast for inflammation, and punctal plugs where tear production is genuinely deficient.

Keratoconus

Progressive thinning and cone-shaped bulging of the cornea, typically starting in the teens or twenties, often in patients who rub their eyes vigorously because of allergy. Causes rapidly increasing irregular astigmatism. Early detection matters enormously, because corneal collagen cross-linking can halt progression — but it cannot reverse what has already occurred.

Allergic Eye Disease

Seasonal and perennial allergic conjunctivitis and vernal keratoconjunctivitis, common in Delhi's children. Intense itching drives eye rubbing, which is itself a major risk factor for keratoconus, so controlling allergy is a corneal protective measure as much as a comfort one.

Corneal Infections (Keratitis)

Bacterial, fungal, viral and rarely acanthamoeba infections, often following minor trauma with vegetable matter or contact lens misuse. A corneal ulcer is an emergency — delay costs permanent scarring in the visual axis. Fungal keratitis in particular is common in India and worsens dramatically if treated with steroids by mistake.

Contact Lens Complications

Hypoxia, giant papillary conjunctivitis, solution toxicity and microbial keratitis from overwear, sleeping in lenses, topping up solution or using tap water. Frequently begins as vague discomfort that patients push through for weeks.

Computer Vision Syndrome

Blink rate drops by more than half during screen work, so the tear film breaks up between blinks. Produces burning, fluctuating vision and headache. Managed with ergonomics, deliberate break routines, a correct working-distance prescription and tear film support.

Corneal Dystrophies & Degenerations

Inherited conditions such as Fuchs endothelial dystrophy and map-dot-fingerprint dystrophy, plus pterygium and band keratopathy. Specular microscopy is essential before any intraocular surgery in these eyes, since a marginal endothelium may decompensate afterwards.

Corneal Scarring & Injury

Following infection, trauma or chemical exposure. Assessment covers visual significance, the state of the limbal stem cells, and whether the eye would benefit from optical correction, a rigid lens or referral for a corneal transplant.

Corneal Topography & Tomography

Maps the curvature of both corneal surfaces and thickness across the entire cornea. This is the definitive test for keratoconus, for screening before refractive surgery, and for planning toric lens implantation in cataract surgery.

Pachymetry

Measures corneal thickness precisely. Essential before laser vision correction, in monitoring keratoconus progression, and in interpreting intraocular pressure readings for glaucoma — thin corneas systematically under-read pressure.

Specular Microscopy

Photographs and counts the endothelial cells on the cornea's inner surface. These cells never regenerate. A low count warns that an eye may not tolerate cataract or refractive surgery well, which is exactly the kind of thing better known before surgery than after.

Tear Film Assessment

Tear break-up time measures film stability; Schirmer's test measures aqueous production; meibomian gland expression assesses oil quality. Together they distinguish evaporative from aqueous-deficient dry eye — different diseases with different treatments.

Anterior Segment OCT

Cross-sectional imaging of cornea, anterior chamber and drainage angle without touching the eye. Used to measure scar depth, assess the angle in glaucoma, and plan corneal procedures.

Corneal Scraping & Culture

In suspected infective keratitis, material is taken from the ulcer for microscopy and culture. This is what distinguishes bacterial from fungal infection and prevents the serious error of giving steroids to a fungal ulcer.

Optical Biometry & IOL Master

Managing dry eye and ocular surface disease

Dry eye is a chronic condition, not an infection that clears in a week. The realistic goal is control — comfortable eyes most of the time, with flare-ups managed as they come — and it usually takes six to twelve weeks of consistent treatment before real improvement is felt.

The single most important behavioural change for most patients is to stop rubbing the eyes. Rubbing worsens allergy, damages the corneal surface, and in susceptible people accelerates keratoconus. It gives ten seconds of relief and hours of inflammation.

  • Use preservative-free lubricants if instilling more than four times a day
  • Warm compresses for five to ten minutes daily for meibomian gland dysfunction
  • Never rub your eyes — it worsens allergy and can accelerate keratoconus
  • Follow the 20-20-20 rule at screens: every 20 minutes, look 20 feet away for 20 seconds
  • Position screens slightly below eye level so the lids cover more of the surface
  • Avoid direct air-conditioning or car vents blowing at your face

Frequently asked questions

Why do my eyes water if they are dry?
It sounds contradictory but it is the classic presentation. Your tear film has two functions — a watery layer for volume and an oily layer that stops it evaporating. When the oil layer fails, the film evaporates too fast, the corneal surface dries out and its dense nerve supply triggers a reflex flood of watery tears. Those reflex tears lack the oil to stay put, so they simply spill over the lid. Treating the underlying gland dysfunction is what actually stops the watering.
Is dry eye curable?
Dry eye is managed rather than cured, in the way asthma or hypertension is managed. With correct identification of the subtype and consistent treatment, most patients achieve comfortable eyes most of the time. Improvement is gradual — expect six to twelve weeks before you feel a real difference, and be sceptical of anyone promising resolution in a week.
What is keratoconus and can it be stopped?
Keratoconus is progressive thinning and cone-shaped bulging of the cornea, usually beginning in the teens or twenties, causing rapidly worsening irregular astigmatism. Corneal collagen cross-linking uses riboflavin and ultraviolet light to strengthen the corneal bonds and halts progression in the great majority of cases. It stops the disease advancing but does not reverse existing distortion, which is exactly why early topographic detection changes the outcome so much.
Why is eye rubbing so harmful?
Vigorous rubbing applies substantial mechanical force to a half-millimetre-thick tissue. In people with a genetic predisposition it is a well-established driver of keratoconus progression. It also worsens allergic inflammation by releasing histamine from mast cells, damages the corneal surface epithelium, and can dislodge a LASIK flap in the early post-operative period. If your eyes itch enough that you cannot stop rubbing, the allergy itself needs treating.
How do I know if my red eye is serious?
Redness with pain, light sensitivity and reduced vision is the combination that matters — it suggests corneal involvement, an ulcer or intraocular inflammation, and needs same-day assessment. Redness that is itchy, both-sided and without vision change is usually allergic or a simple conjunctivitis. Any red painful eye in a contact lens wearer should be treated as an emergency until proven otherwise, and the lens must not be reinserted.
Can I use over-the-counter redness-relief drops?
Vasoconstrictor drops marketed for redness are best avoided for regular use. They constrict surface blood vessels and mask the appearance of redness without treating the cause, and on stopping they produce rebound redness that is worse than the original, which drives dependence. Lubricating drops, ideally preservative free if used frequently, are the safe alternative.
What causes eye strain from computer work?
Blink rate falls from about fifteen per minute to under seven during concentrated screen work, so the tear film breaks up between blinks and the surface dries. Add sustained focusing effort at a fixed near distance, screen glare, and an air-conditioned room, and you have the classic picture of burning, fluctuating vision and end-of-day headache. It is genuinely treatable with tear film management, ergonomics and often a correct working-distance prescription.
Do I need a corneal transplant?
Corneal transplantation is reserved for corneas that have lost transparency in the visual axis from dense scarring, advanced dystrophy or endothelial failure, or that have become too irregular for any optical correction. Most corneal conditions never reach this point. Where it is genuinely indicated, modern selective techniques such as DALK and DSEK replace only the affected corneal layer rather than the full thickness, with faster recovery and lower rejection risk than traditional full-thickness grafts.
